 What is your financial policy? Payment is accepted in full at the time of service, unless other arrangements are made. We accept cash, checks, VISA, MasterCard, and Discover.
What insurance do you accept? Please contact our office for a list of currently accepted insurers.
What are your office hours? We are open Monday through Thursday from 8:30AM to 4:00PM and on Friday from 8:30AM to 12:00PM. Dr. Cornelison is in the office Monday through Thursday and in surgery on Friday. Dr. Tager is in the office on Monday and Tuesday.
What paperwork should I bring to my appointment? Please bring your medical and patient registration forms. Be sure to bring your insurance card, referral if required by your insurer, and workers compensation authorization if needed. |
